GBPCA is engaged in supporting their members and the entire safety net medical home community in Nevada regarding practice transformation.
GBPCA provides technical assistance and training, working to improve the health status and quality of health care provided to patients. Specifically, GBPCA is interested in working to inform, educate and provide varying forms of technical assistance to FQHCs and other organizations who are interested in seeking recognition as a patient centered medical home (PCMH).
Our quality improvement programs support our mission to increase health equity and access to comprehensive health care for low-income and vulnerable people in Nevada.
The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. PCMH is emerging as a key strategy to improve health outcomes, reduce total costs, and strengthen primary care. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
